Agreement to
Mediate
If you wish to engage in mediation or facilitation of your dispute, you will be required to sign an Agreement to Mediate.
Agreement to Mediation-Arbitration
If you wish to go ahead with Med-Arb you will be required to sign an Agreement to Mediation- Arbitration.
